INSURANCE:
As a courtesy to our patients, North Pole Physical Therapy files primary insurance and secondary insurance. We accept Medicare and Medicaid. We are also preferred providers for Blue Cross as well as network providers for Tricare.

Insurance coverage for physical therapy varies depending on your insurance company. Some companies require pre-certification and some limit the number of physical therapy visits. We recommend that you contact your insurance company to verify the specific requirements of your coverage. You should expect to pay your deductible if it has not yet been met and any co-insurance or co-pays for which you are responsible. It is important to remember that your coverage is a contract between you and your insurance company. North Pole Physical Therapy will do all that it can to help you in receiving coverage for services but ultimately you are responsible for your bill. We will gladly accept your co-payment at each office visit to avoid having to remit co-payment balances after the insurance has paid.

Statements will be mailed to you at the beginning of each month if there is a patient balance on your account. Custom Medical Billing Services (CMBS) is contracted by North Pole Physical Therapy to perform billing services. Most claims are filed electronically with the insurance company. You can call CMBS toll free at 1-866-774-5600. They are the experts on billing and your account. Of course you can also call our office at 488-4978 with any questions on your account. You can fax to CMBS at 1-866-756-7549. The office for CMBS is located in Anchorage.
